
Since 2017, USR Cares has helped more than 60 families, individuals and couples who requested our assistance when faced with a crisis.
We provide support for individuals and families who request assistance after experiencing a temporary crisis. This support comes in many meaningful forms, both financial and non-financial, including initial case assessment and care planning, food and meal deliveries, transportation to medical appointments, case management support, caregiver support, relocation services, recommendations and referrals for home repairs and minor house modifications to address physical needs.
Our Care and Crisis team assists residents experiencing a crisis or who are in need by providing and navigating immediate solutions.
Examples of this assistance include:
Coordinating and covering the cost, where appropriate, of short term care management services and acting as a liaison with hospital staff and social workers
Stocking the refrigerator and arrange for needed cleaning services before a resident returns from the hospital
Responding quickly to USR Police concerns about residents at risk
Working with community organizations to coordinate support for various USR residents yard work and house-specific needs
Assisting an at-risk young man with a challenging home environment by providing transportation, emotional support, assistance with college applications, providing financial support for his critical needs and collecting supplies as he achieved a significant milestone and embarked upon his collegiate pursuits.
Connecting and, where appropriate, paying the cost of minor repairs and upgrades to address physical needs
Helping coordinate and facilitate relocation and transitions to senior care facilities
Assisting young families with needed services from clinical to social issues.
Helping seniors navigate how to remain in their homes safely.
